To register for treatment at Godeshöhe, please complete the following forms and send them to the relevant fax number:

• Registration form/discharge summary or post-discharge rehabilitation application
• Barthel form

If you wish to apply for the cost of your treatment to be covered by your health fund or insurance company, then you should fax them your application forms at the same time that send them to us.

Presenting a written guarantee confirming reimbursement of expenses will enable prompt admission.
